Severity of cognitive impairment affects interrater reliability in MMSE scoring. It is recommended that training on scoring conventions reflect the protocol driven degree of severity required for the study population. The use of standardized scoring conventions should be required for raters in order to improve data accuracy when rating moderately to severely impaired subjects. In addition, applied training practices should be included in training sessions.
